Student Sports Physical Exam Requirement
Students that participate in the Elementary & TK-8 Athletics program must have a sports physical examination form on file that is completed/signed by the parent/guardian and the student's health care provider/doctor. To ensure that the student's health status is up to date, the sports physical examination form is valid for 1 (one) year from the date when it is signed by the health care provider/doctor. The form should be filled out completely and accurately by both the parent and the health care provider/doctor. The completed form should be submitted to the Athletics Coordinator at the student's school site. If there are significant changes in the student's health or if the student experiences a new medical issue(s) during the year, an updated form or additional documentation may be needed.
